In three out of four of my latest games my king has died with no heirs and it was game over. Am I doing something wrong or is it just the luck of the draw.
if you have VI then theres this 56-bug where your king always dies when he turns 56.
you can always type .unfreeze. its a cheat to have heirs http://www.totalwar.org/forum/non-cgi/emoticons/smile.gif
Aaaah, I thought the cheat was .freeze. http://www.totalwar.org/forum/non-cgi/emoticons/tongue.gif Maybe that's just princesses...

When you say he died without heirs...did he ever have any? (A polite way of asking: did you accidentally get his heirs killed in battle, etc?) Other than that, there is the age 56 bug, but if you are careful you should not run out of heirs. I might have lost one game this way in VI.

I don't see where the problem is...my king is always known for his great virility...maybe you're marrying your princes to the wrong women. http://www.totalwar.org/forum/non-cgi/emoticons/biggrin.gif

Tip: Always marry your princes off when they come of age, hunt down those princesses and get them to marry your princes, this will help with getting heirs sooner. Additionally, if your king doesnt have any heirs, you better not get him killed off in a fight or anything until he does. Also additionally, an heir doesnt always have to be a son of the king, it could be the kings brother if the kings son hasnt come of age by the time the king dies. This can be really bad because sometimes the king dies and you could end up having a successor who is only 3 years younger and unmarried, essentially putting you in the crapper.
