Easiest question first:
The game lasts until 1453 unless you conquer everybody before that date.
You need to build a brothel to train spies. The prerequsites of a brothel are a tavern and a castle. A spy can do the following:
Increase loyalty in one of your provinces.
Reduce loyalty in an enemy province.
Protect one of your provinces against enemies spies & assassins.
Frame one of your generals for treason.
Reveal hidden vices of an enemy general.
Attempt to open the gates of a besieged castle, allowing your troops to gain enterance in the middle of the night.
For dealing with rebellions: build happiness increasing buildings like churches and watchtowers, increase the size of your garrisons, reduce taxes and use spies as an internal secret police thus stamping out potential rebellions. Use the shift key every turn to see what the loyalty of your provinces is - green is good, yellow and red are bad.
I think there's a lot of stuff in the guides section about increasing loyalty in your empire. The larger your empire gets, the more problems you will have with rebellions.
spies you need to have a brothel which requires a tavern and a castle.
they increase loyalty.
try using religious agents to convert the populace and stuff peasants into non borders to boost loyalty.
if only 1/4 of the map, you shouldn't have problems with too many rebellions
try to have your king near the geographic center fo the empire and not stuck on some forsaken island without naval link.
