How can I do this? Do I have to attack the enemy ships with my ships in order to clear the blockade?

Nope you can have 30 cog in a region but if an enemy has just 1 dhow in the same region it will be blockaded http://www.totalwar.org/forum/non-cgi/emoticons/gc-inquisitive.gif

Aye it took me a while to realise that one as well. Now i always make sure im king o' the seas by having loads of ships outside my enemys harbours.

...and you better make sure you attack because if you don't the chances that you'll lose your fleet increase dramatically.
I've seen this in my most recent games. Every time my trade fleet was sneak-attacked by another faction, I lost an average of 4 to 5 stacks of ships in the attack.
When I regrouped the surviving ships and started to systematically attack the backstabber's ships, I won 9/10 battles... http://www.totalwar.org/forum/non-cgi/emoticons/gc-confused.gif
But it's well-known that MTW's naval AI isn't that great.

Has anyone ever tried to mod some form of naval battle in MTW? must be possibel for some of the modders around this place judging on the high quality of work that goes on

If you can manage to have zero contact (no land adjacencies, and no ships in the same sea region - but adjacent is ok) for a turn, you will become at peace with that faction. This is an important option sometimes.
It's an especially great trick to stop being at war with e.g. folks on islands, so you can trade with them instead. (You make a lot more money having those crappy Med islands to trade to, as long as they have a port, than you ever would, holding them.) Also of course, you stop having to worry about the hassle of them pumping out a ship every now and then. I've seen the AI cheat and make ships even when they have no shipwright buildings. This is on Expert; dunno if that makes a difference.
Note that it is sometimes tricky to tell if you've done enough to be 'away' from an enemy. This is especially true of rebels. All rebels on the map are at war with you, if any one of them is. This includes rebels holed up in a castle (because you just killed somebody's king). There's no indicator of being at war with rebels on the Alliance popup, but you can check it by turning on Tooltips and holding the cursor over some of their land. Or, see if you're trading with them via the Econ popup, when you should be.
