OK, for the first time I am actually doing well, and have had a plan and am beginning to take over as the SHIMAZU.
I have 4 geishas of differing honor values.
When an enemy geisha seeks out one of my geishas or I send one of my geishas after an enemy geisha. The vignette "mini video" always shows both geishas dying, but then I get a success message and sometimes my geisha lives and sometimes she is gone. AM I MISSING something here, bug maybe?

Hmm ... whenever a Geisha duel has occurred in my games, both died. In Geisha wars, I've found that their honor level only matters in that the percentage chance of either Geisha's attempt to assassinate the other that turn are higher if her honor is higher. If only one is trying to kill the other, then there is one chance per turn (assuming the target is in range). If both are trying to kill each other, then each gets a chance. Regardless of which is actually attacking, if she is "successful," both die.
Because of this, I do these things:
Always try and be the first to produce Geishas, and keep cranking them out as quickly as possibly. I usually have one province dedicated to their training, and keep the queue full until I have around six.
Once a geisha is produced, kill with her every turn if possible, to raised her honor quickly. Emissaries are best, particularly those with honor as they are still easy for her to kill. If none are around, then I target 1- to 3-star generals depending on her honor level. I don't usually target 4- to 6-star generals until she reaches rank 4-6 herself (unless the situation demands that I try).
Until she exceeds Honor 2, the chances of her being killed by ninja are still high if she ventures outside of protected zones (your own provinces with shinobi/ninja counter-intelligence or Border Forts). If you suspect enemy ninja are about, one trick is to place her outside of your protected area for a turn to get their attention (initiate their attack on her), then move her back into a heavily protected province and watch the ninjas come and get caught.
When an enemy geisha appears, I immediately target her with my lowest ranking geisha. Since both die, I don't waste high honor geisha on enemy geisha unless I have no other choice. Since I usually notice them right away, the enemy geisha are Honor 0 and easy for my low-level geisha to kill. I also swarm her with ninja if she comes into my provinces and is honor 2 or less (but I stop them if she enters enemy territory, to avoid them getting caught by enemy counter-intelligence).
If you send multiple assassins after a geisha and manage to kill her, be careful on the turn immediately following her demise. If you try to pick up any assassin (ninja are particularly bad for this) that is still shown as moving to attack (the icon is different when attacking than it is when just patrolling), the game will crash to the desktop. I'm not sure if picking up a geisha will do this, but I know ninja will. To avoid the consequences of these kinds of crashes, I always Quicksave (Control+S) before hitting the Next Turn button, so if a crash occurs I will only lose the work done that turn.

I only ever used geisha to attak heirs or dianamo's (pardon my spelling)
And i sent assains after geishas.
They seem to make Killing off factions easy, so i tend not to use em of late
Haven't used geisha's very much yet, but I remember them both dying when a geishafight appeared. I prefer just fighting and qonquering Japan, coz the battles are so much fun in campaigns. ~:)
THANX for all of the answers Gentlemen. I like the info about picking off nosey ninjas sniffing after my geishas.
This game is just sooooo deep.
