Where do babies come from?

I was just wondering that if your Generals where on the campaign map(Out of a settlement) or staying in a fort to prevent any corruption traits from reaching them. Can their wives still produce babies? Or does your General have to be in a Settlement for the wives to produce babies?

Those who have sunk more time than I into discovering those types of inner workings of RTW came to the conclusion, that a family member has to be in a city to get marital nookie.
