If he isn't married, try to snatch a princess from somewhere and hope he lives long enough for his son to mature.
If he is married, hope for the best. Maybe his wife might give him a son.
The only alternative that I can think of is engineering a civil war and backing the rebels. It will cause a major setback for your sultanate (or whatever it's called) but at least the chances are good the leader of the revolt will be fertile.
If you don't mind using cheats, you can type .unfreeze. and your wife will give birth to a son the next turn. Then you have to hold on and hope your current sultan makes it til he's at least 62. ~:)
EDIT: Ack, I can't spell!

With a son on the way at 46 hell grow in time as most kings kick the bucket around 65. I wonder if typing unfreeze multiple times gets you more children. You could get a full stack of princes, lol.

It will cause more princes to be born, but you can still only have like 8 heirs at a time.

Does that mean you willl start to get royal knights that are of royal blood?

No, you can only use the cheat until you have 8 male heirs born (or is it six?), then it stops working.

My king is in the same boat. He ruled for about 20 years producing five children but no son. Finally at 48 an heir was born. I hope he lives long enough to save my kingdom.

My record is 9 daughters in a row before the first son was born. As it was the unpatched game (v 2.0 with its 56 year bug), that was my only campaign I've lost because lacking adult hiers.

Probably six, as that the maximum number of male hiers you can have at the same time in the regular game.

would have been nice to have a full stack of princes though.
