Every 10 - 20 turns a provience randomly will become mind, is that meant to happen?

If you're playing as Pahlava or the Koinon Hellenon.. Get used to it.
It's down to the faction cultures.. when a city rebels it goes to the faction with the closest culture. For many cities around the med it's KH, while in the Arabian peninsula they tend to go to Parthia.

There are two main things that affect a city revolting to a faction: the default_faction in descr_regions.txt, and the faction_creator in descr_strat.txt. The faction_creator determines things like the city's appearance, while the default_faction (I believe) is who the city would prefer to revolt to. I believe both have at least some influence over cities revolting, at least early in the game.
From what I've seen, the effects of these are less with the 1.5 patch, though I'm not 100% sure.

Which is on reason why I believe that the new faction is in Arabia...KH makes plenty of sense, by what did Yemen care about the Pahlava?

Parthia being the founding faction for Arabia kind of makes since, you want it to look eastern, not the same culture as Ptoli, and you have to choose a faction to found every town. Revolts are an unfortunate side-effect.
Yemen and a persian empire didn't meet until several centurys into AD when the sassinids conquered the east and south shores of arabia.

Yes I know that very well. And it always happens when you have just tried very hard to make an alliance with them. The same happens with the Sassanids and that Arminian province. It's creator is Eastern Roman so if you just got a hard earned ceasefire (or even an alliance) with the Sassanids your back at WAR and this time they don't say ceasefire!
Perhaps Arabia should revolt back to the Rebels! However I am not sure if the slave faction can be the faction creator, although the team could try it...
