I have been playing MTW2 for nearly a year and I have yet to see a civil war occur to any faction. In MTW it has occured when you took enough provinces from a faction or you had enough disloyal generals plus a weak king. Made it easier as you only needed to get civil war to cripple them, not wipe them out. Has a civil war ever happened in MTW2?

I never seen it happen either. I don't even know what the loyatly rating does in MT:W2. I think it was a good idea in the original to have to watch your general's loyalty and give them titles to keep them happy. It added another dimension to the game....

civil war is simply not in the game.

loyalty affects whether that general will rebel or get bribed by the AI (not that the latter has ever happened to me)

Yeah I miss the Civil wars and the Reemerging Factions events. They certainly kept you on your toes.

thats because its simply too easy to gain authority. Either start up some Assassin/Spying or kill em off for the next in line, and a good authority can be achieved in as little as 1 turn.
