Is there a minimum "run up" you need in order to get the charge bonus? If you are standing still and then right before the other unit closes on you you attack, do you still get the charge bonus?
The unit has to be properly formed and there is a bit of a run up. Generally if you mouse over the unit you will see it's status which is how you can be sure if it's charging or not. I think charging is called "attacking" and melee is referred to as simply "fighting".
It's difficult to quantify exactly, but I generally seem to need 50-100 yards in order to activate a unit's Charge ability. If they're closer to an enemy than that, they usually only fight with their normal Melee ability and cannot "charge" per se.
For fast cav, if you're unit is stationary and facing the charging enemy unit, you can adequately countercharge from 40 meters. If the enemy cav unit is coming in obliquely, you'd best initiate the countercharge before it gets within 80 meters, because your unit has to reform to face the enemy unit. You should double click on the enemy unit because single click may not reform your unit fast enough. Forty meters is the distance from which a cav unit starts to accelerate to charge speed. You'll see the individual cavalrymen lower their lances as they achieve charge momentum, and that's the sign that their charge bonus is activated. For infantry, I would cut these distances in half since infantry moves at about half the speed of cavalry. The charge activation distance for infantry is 30 meters.

The battlefield consists of tiles which measure 40*40 meters.
Two and a half tiles are 100 meters.
It's not that easy to see the individual tiles, depends on the map. There's also a map in one of the mappacks that only shows a grid and makes measuring easy.
Yuuki may correct me here, but I think that a 60 men unit in 3 ranks has a width of one texture tile (40 meters).
