LOL, you fought against H or even VH difficulty don't you? and bet you didn't have government inside your town, or your government has no / bad security sense..... Actually, the AI uses spy exstensively, and remember : YOU DIDN'T GET ANY MESSAGE IF A SPY INFILTRATE YOUR SETTLEMENTS, you get the message if he was either killed or expelled, that's why your settlement's gate will be opened... (ai spy often get very good subterfuge)

IMHO in EB the message if a enemy spy has opened the gates is disabled. Correct me if I'm wrong.
You can put a spy in your own city to decrease the chance that an enemy spy can slip in your town. Also traits who improve the popular security are helpful.
If you send your spy to an enemy city, a number in the bottom will appear, showing how high the chance is to open the gates if you attack the city. Send more spies to increase this number.

But I remember in RTW that you can't deploy your troops if a spy infiltrated your city. They just came running at you.

Not exactly helpful, since neither difficulty level nor government affect the chance of successful infiltration. The only way of expelling an enemy spy is placing on of your own in the city; and if the enemy spy is good even this may not work.
Also, there IIRC is a bug that will cause the gates to be permanently open even after the spy is expelled. Try to search for it in the forum; although I don't think a solution for it was found. It's something in R:TW's core-engine: the EB team cannot fix it.
