In some cities, it seems to be possible to build 2 ports (Capua), in most others only one port type can be built. Is this a bug or on purpose?

It's not a bug. Only few cities can build fleets, while all (or most) can build trade ports

And since Naval ports and fleets are incredibly expensive, build only a few. As Rome I build Capua, then the one in Spain when I take that and Rhodes when I take that. No need to waste money on more than 1 centre, 1 East, 1 West, and base a fleet each place.
