"Unknown Building" appearing at random
I've no idea what the cause of this is, though I suspect it's something to do with the reading/transcribing of descr_strat.txt as a new game is built, but it isn't a consistent error. It seems there is always somewhere with at least one of these, but it isn't the same place every time. In my present game, Halikarnassos has one in place of its Hellenistic Poleis building. In another game I generated, Trapezous had one.
In this game I just generated, two places had them:

As I say, it seems to be completely random where it will appear from one new game to the next.

This is actually completely normal - it's the game emulating insufficient information that you have of enemy settlements. If you move a spy with a sufficient level next to the settlement you should be able to get the information of those buildings.

I was using the generic script/perfect spy to have a look at whether buildings were spawning correctly - so this means they are there?

I have never used the 'perfect spy' command so I cannot comment on it's efficiency - but the building exists. The same happens with individual units in enemy army stacks that you do not have a good view of. The question mark is basically a place holder for missing (game play wise, not actual) information.
