Have you aktivated the script every time you start a new campaign or load a savegame. You start it by clicking on show me how on the advisor who always pops up if you click on a settlement.

The Polybian Reforms are coming after 242 if you have conquered 2 of these: Patavium, Mediolanum, Bononia, Segesta
and 2 of these: Lilibyum, Syrakus, Messina

You have fallen into the huge trap that many new EB players fall into. Sorry to break it to you, but your old games are now bugged. Playable, but bugged. To stop your problem occurring in the future:
- First start a campaign as your chosen faction.
- Then click on a settlement.
- You will see the advisor. Click on his/her portrait.
- Then click the Show Me How button which is the one that looks like a blackboard with a piece of chalk on it. This activates the background script which is essential in playing EB to it's full.
- Play on as normal. Repeat these actions whenever you load a saved game.

1) Not for the Polybian reforms. The number of battles only plays a role for Marian and Augustean reforms.
2) The recommended difficulty settings are VH campaign and M battles. These are the settings the game is balanced at. Lower than VH campaign will make the rebel A.I. passive; higher than M battle will give the A.I.'s troops unbalancing stat bonuses. They are not obligatory, though, and several players prefer H battles because of the extra challenge.
3) Same as the first answer: this is not important for the Polybian reform.
