In my experience the best way to prevent armies rebelling is to ensure your faction leader has a high authority; you can do this by winning lots of battles with him, and by making sure he has high either dread or chivalry. Also, I don't ever accept "man-of-the-hour" generals if they have any traits like "Speaks of Loyalty".
Also seems that the further you are from your capital city or king, the likelihood of revolt from an officer-less army increases. I as well do not accept either Man of the Hour or new family members with negative loyalty traits at least if I can help it.

Off topic but an update on a previous question I asked about Man of the Hour and militia units. I can confirm that indeed raw militia CAN produce new officers as it happened yesterday on a Danish campaign. Whats more it was a rebel stack of only 3 units directly outside of my city which produced an officer for me.
