What is exactly a large battle, and has the enemy armies have to count with catafract units, or just have the unit among their options?¿

There have to be cataphracts in their stack as a unit. A large battle is probably 10 units or more, to be safe.

Unfortunately we cannot detect the actual units, so any great enough force with a decent amount of cavalry will do.
