In BI when you recruit a General he gets a portrait, but when I recruit one in RTW I get a "generals bodyguard" unit card! Am I doing something wrong?

You won't see a portait until the unit appears on the campaign map, so just adding it to the recruitment queue won't do anything.
No, when the unit is done recruiting it's still just a "general's bodyguard" unit! No portrait...
Does the unit have the general_unit attribute in its EDU entry?

Do any other units recruitable by macedon and the seleucids also have the general_unit attribute? If there are more than one, the game is probably considering the last one in the file to be the general, and thinking the first is just another ordinary unit.
Also, is it before or after the Marian Reforms? The generals before the Reforms use the unit with the general_unit attribute. Generals after the Reforms use the unit with the general_unit_upgrade attribute.

No other units have general units attribute... Could the problem be where there recruited? Or maybe it's the soldier type?

The place they are recruited from will not matter. All the Soldier line does is tell the game what model and skin to use for them on the battle map, so that is not the issue either.
Do you have RTW 1.5? Maybe this does not work on earlier versions?

I'm currently playing "Rome Total Realism" which patches from 1.5 I believe...
That depends on which RTR you have. The newer version uses 1.5, but there is an old version out there for 1.2. Start the game, go to Options, and you will see your version number in the upper right corner.
