Far too many ultra-geeks (not nerds) who lust after such things. This is not the first time video game or cartoon characters appeared in Playboy IIRC. Although I did think such mainstream franchise as The Simpsons would not do this. That said, the quality of Simpsons declined from its first few seasons, and I have stopped watching them long, long time ago.

Simpsons fans are bizzarre, to put it lightly. They are far too hardcore and obsessed for their own good. (not a very child-friendly episode, BTW)

Until pregnancy, women's breasts are pretty much all fat. Only then does the ductal tissue form. Hence why the breasts become swollen and painful.
