I conquered Jerusalem as the Seleucid Empire and noticed that the gates are always broken and open. The walls are repaired and even when I upgrade to the next larger wall, the gates stay open.
How can I fix this?

You have an enemy spy in your settlement who is opening the gates when the enemy attacks. Put some spies in your city and he should get caught.
I thought of that first, as well. But the gates look damaged - would that be the case if a spy were in the settlement?
