You have to make good use of temples. Decide where your training centers are going to be and build temples that give exp bonuses and/or equipment upgrades in those places. The further the city from the capital, the more important it is to use law and order temples. The biggest key is to achieve ZPG (Zero Population Growth). If you can do that, then you halt squalor and unrest. Pay attention to ancillaries, as well. Ancillaries that promote order and reduce squalor are at a premium. Use governors with high influence...never mind if they don't have management skills. You are probably making more money than you can spend at this point anyways, so keeping order takes priority. Your best field generals are good candidates for this. I used to keep my premium field commanders out fighting the enemy until they died of old age. Now, I retire them at around age 50 if they have accumulated high prestige (which they often do), as they are the perfect governors for troublesome cities. More then once, such a governor has bought me enough time to make all the required changes and developments to a very large, newly conquered city. But that's just me....And as pointed out, try to locate your capital in a central location to reduce the distance-to-capital penalty.
I've got some screenies to illustrate for troublesome places like Jerusalem, Carthage, Alexandria, etc., if you'd like......

Well...I wouldn't go and retire them all.because all my great and influente generales are facing the roman armies in europeI almost always will pair an up-and-coming general with my aging centurions so that they can accumulate an experienced bodyguard, and can take the veterans ancillaries that boost leadership/movement/honor etc, when I retire the man to be a city governor.
As I said, that's just my style, and it works for meThe number one thing to strive for is ZPG. Once you get that, you might not even need a governor. Many times I've gotten away without one even in high populace cities like Carthage and Alexandria.

you'll just need to put simple half-flagged garrisons on each city, regardless of the Unit. well, I do that and never get a single settlement rebelling for some 15 years in RTW vanilla
