Go to settings at the top right of your screen, look for the my settings box and chose edit avatar. Its that simple.

I'm pretty sure you can't although I remember it being discussed a while ago. PM a mod to make sure though.

You can in a certain way. Most staff have custom avatars. Certain other members have gotten them on base of merit. These cases are, however, very rare.
If you're talking about an avatar you upload yourself, you could read this.
