I used to hotkey everything 5-7 years ago. Ctrl+Shift+W = Word, Ctrl+Shift+I = Internet Explorer, etc. and so it goes.
Nowadays I hide the start menu and dock because they just take up precious real estate. In Mas OSX I run Quicksilver and for Windows I run an app called Launchy. Basically it lets me hotkey Ctrl+Space to start a pop-up and I can just start typing the name of the app or function I want. It does partial names and indexes installed applications on the fly. I agree that the start menu (and Mac Dock) is obsolete. Unfortunately they keep tying key tasks to that bar in Windows.
Windows 8 will be interesting...
