Friendly units will not make melee attacks against each other... The only ways I know to get friendly kill are firstly the obvious missile fire and secondly elephants running amoke (chariots can also run amoke but don't know if they can cause friendly casualties)... Note that even if you charge chariots or elephants through friendly units (not running amoke) they will just knock a lot of people down, not kill anyone...

Are you sure your general was not close to the enemy? I have seen generals being the first to die at the points of an enemy phalanx (not even attacking it, just getting to close when passing in front of it)...
