    I just discovered that I misread the question. The character came of age long after the Carthaginians were exterminated. In that case, there are two possibilities: either he got it from his father, or he decided he hated the Carthaginians based on history lessons from before he was 16 (presumably). The former possibility is vastly more likely, assuming his father did either hate or fear Carthaginians:

    As for Doubtful Courage, there's unfortunately no way to get rid of it (I think). The Coward trait inexplicably has a NoGoingBackLevel of 2, which is exactly the threshold of Doubtful Courage. Unless a NoGoingBackLevel does something unintuitive, that means you're stuck. Most NoGoingBackLevels are higher. If you want to raise the level, go to data\export_descr_character_traits.txt, search for "trait coward", and edit the line that says "NoGoingBackLevel 2." Setting it to 6 or 8 (Craven Coward and Ruled by Fear, respectively) would be more reasonable, or you could delete it altogether.
