Prior to 1.3, I THINK you had to send your general back to a city in order to be cured, which created enormous problems if he contracted the plague while not in a city. Does anyone know whether sending him to a city with a public health temple (Askeplios, etc.) will help shorten the duration and/or increase the likelihood of his survival? What about a city with sewers, public baths, etc.?
Intentionally spreading the plague is great if you control Egypt, where the citizens breed like lice. I don't use it offensively, though. Not sporting, really. It cuts both ways, regardless. After 1.3, with the AI's increased emphasis on using diplomats/spies, I had the same plague-infested uber spy infect my capital city so many times that I finally gave up.
