Gameplay enhancents don't alter the campaign, but change some small things to make the game better. Examples are Player1's bugfixer (which is a sort of unofficial patch) and DarthFormations (improves A.I. on the battlefield). I would recommend both, Darthformations where designed for previous versions of the game and do not seem to work well with the latest patches.

Total modifications with the same subject are Rome: Total Realism and Europa Barbarorum. The games still deal with the rise of Rome, but both strive to make the game more realistic, challenging and fun. Both are still for R:TW 1.2, but they will be converted to later versions in the future. R:TR is far more finished, but EB is the most challenging and historically accurate. Like Simon said, EB is still in open beta, so I don't recommend it unless you don't mind some bugs and unfinished or unbalanced features. There is also Darthmod (as oposed to Darthformations), which strongly increases A.I. performance on the battlefield, but far less popular than either R:TR or EB.

As for mods with a different subject: there are far too many too name. Have a look in the hosted mod forum to see a few of the more popular projects.

All I have mentioned here are the most popular modifications, there are many more out there. If you want to know about them, you can try browing the modding forums of the Org and the TW center.
