Most annoying sound in a game?
What ingame sound unnerves/annoys you the most? I'd go with the sound of the soliders issuing commands to eachother in Half-Life I, not because it was scary, but becasue it meant that you we're going to spend 10 minutes trying to out maneuver 5 commandos(The AI in Half-Life I was amazing, considering that it the game is like 6 years old)

The music in pretty much every game except Star Wars games.
The occasional sound "YEEEEAAAARRRRGGHHHH" in Starcraft (+ same sample is used in a zillion radio/TV ads, many songs and even some movies, seems to be some kind of a running gag between sound mixers)
maybe those guys should be doing something more useful...

The Terran Academy sound (in Starcraft) was particularly grating.
Some of the Zerg units, too.

Ah man! I remeber that! First time I played I didn't bother to read the manual and after about a minute it start going off and it took me about 10 minutes to figure out how to allocate plebs to my various projects.

I tend to hate the constipation sounds that play in almost 100% of games when your male character is performing any sort of jump.

Virtually all the main party's speech samples in Baldur's Gate - especially the ones with grating Brooklyn Jewish accents. In fact I had to get rid of one pretty good character because her know-it-all "Jewish mother" accent/attitude so got on my nerves.
But other than that, mainly because the speech samples were so infuriatingly repetitive and witless.
Oh, the speech samples of those perpetually annoyed incidental female characters in Morrowind also really got on my willies too.
