There ought to be more plagues in cities, especially in large ones. In Rome, I was sick of evacuating major cities, allowing them to rebel, then retaking and exterminating the population.

Plagues frequently occured in the Middle Ages, and this would help control the population of cities more.

You have to remember this is a game and controlloing cities is actually toned down from medieval times. Also the game engine has been heavily tweaked according to the dev's. From dev statements plagues will be devastating but not to the degree of impact they had in real-time. And what was so hard about controlling cities in RTW anyways, even on very hard difficulty.

More plagues just for the sake of population control? No thanks. I like having large cities and for whatever reason I have never had a problem keeping them under control.

I hope they're about as worse as exterminating the population, but occur because of non-random historical events. This way you can plan for it and you actually notice their effects (unlike any current implementation). It also presents an extra challenge, but requires more time to make it an option for people who don't want it. This does not have priority. IMHO, immersion is pretty good already, AI and game speeds require more work.
