    Is it possible to create new portraits from scratch? Is so, how is this done?

    More a case of no time, I should think ;)

    You simply replace the images in the relevant subfolders of the UI folder for each culture. Not a quick task as you will need many variations (dead, old, young) for both cards and portrait cards, and the civs too. Oh, any you'll need to alpha layer all the images if you want them to look good in-game. Make sure your paint program has batch processing ;)

    Take a look at the files of a mod that has edited or created its own portraits.

    Various versions of RTW/BI will work in different ways so go for the Mod that is based on the version you wish to modify when examining the files.
