It could be a city rebelling without access to any rebel units
But if that was the case, wouldn't the game only CTD on the Rebels turn?
There are two cases, one where a city revolts to the rebels and one where it revolts to a faction.

When it revolts to the rebels it draws units from any that are buildable in that city first and if there aren't any it gets them from the list of units for random spawn rebel in that province.

When it revolts to an AI faction (or your faction) it does the same thing except if there are no units buildable in the city then the game pulls units from the local mercenary pool instead of the local rebel list.

In the second case at least, if there are zero units available (no unit buildings and an empty merc pool), then the city revolts with no problem. The city just has no garrison after the revolt. So I think the problem is, sometimes at least, some kind of incompatibility between either a merc unit being spawned for a revolt and the faction it is being spawned for.
    It's not a map.

Well, that is good news then nikolai. There were some of those incompaibility issues with mercs and stuff that Aymar has been getting rid of. So if that could possibly lead to problems, maybe we will have a reduction of potential CTD's that we weren't even planning on.

It happened again and this time I used the Romans. I took all of Italy and Sicily and was planning to move out of the penisula and now I can't even reload it, why is this such a problem?

Why aren't you able to reload the game?

Yeah, hopefully. I've had this problem with lots of different mods and my own but it is so random and often very difficult to tell which city has revolted.

I've been testing this a bit as it seems a problem for a lot of mods with very expanded unit lists (as more room for errors). I've been putting whatever I find out here:

I have no idea, I click Continue Campaign or Load Game and it goes to the loading screen with the quote and interesting background and before it starts to load it immediatly then crashed to desktop. I'd like to play my save games but I can't, and I'm afraid to start anymore.
