There's Fourth Age: Total War, and there's Lord of the Rings: Total War.

Fourth Age - Total War is out and downloadable now, along with a Corsair Invasion Expansion Pack.

Lord of the Rings - Total War is out as a Beta (with just custom battles), but some fans have added the Middle Earth - Total War map to create a campaign (note: this is non-official)

Middle Earth - Total War There is a downloadable release on the LotR TW site, I'm not sure if work is continuing...

Middle-earth: Total War is based strictly on the lore and writings by JRR Tolkien and set at the end of the Third Age, at the time of The Lord of the Rings book(s).

The Lord of the Rings: Total War is based on the films by Peter Jackson so some elements are contradictory to Tolkien lore. This is set during the War of the Ring, just like the films.

The Fourth Age: Total War is based on the Fourth Age of Middle-earth. Although not based on any particular story by Tolkien, it draws its sources strictly from the writings of JRR Tolkien.

Keep in mind, though, the current releases of LotR TW and Middle Earth TW are beta versions for RTW 1.2
So if you have BI installed, or the Gold edition, these will not work for you.
