Can you remove the card from your system and try to identify the exact model, revision etc by googling the part number on the card?

I might be able to do that few days later when I get in touch with my PC. BTW, I don't know how's revision and part number can help?? Maybe you could be more explicit on this?

The revision and part number will help to discern, via a google search, if anyone else has had similar problem with that exact board. This can save alot of time. It also ensure that you are downloading the correct driver, which as you've been having problems with drivers is another issue.

    Power supply issues maybe?

I can't imagine it would be a power issue. The amount of power consumed by a 56k internal pci windmodem is negligible. Soundcards aren't big power hogs either. The biggest consumers are usually your Graphics card, SDRAM, HDD's and of course the CPU itself.

What's the latest with this Tran?

Okay I am a loss here, there are lots of numbers here on the modem card, so I guess I'll just post every numbers that are listed and maybe you can help me identify which one is the important numbers and perhaps offer a fresh solution

Spoiler Alert, click show to read:
    Back part of modem:

    Tested To Comply
    With FCC Standard
    FOR HOME OFFICE USE
This devise complies with part 68 FCC rules
    FCC REG No:3P7MM08BDFM562IS
REN AC 0.8(B) USOC JACK RJ11C

    D-Link

    H/W Ver.:D1
    DFM-5621S/SG

    Complies With
    IDA Standards

then on the right part of back

    22 94V-0
    060 1

    Front part of modem:

D-Link DFM-5621S/SG, based on Connexant CX11252-41Z chipset. That winmodem is supported natively by XP and unlikely to be the problem. The soundcard is probably where you need to be looking, not the modem.

I'm assuming, from your previous posts, that the soundcard works fine with the modem removed?
