I've had this before, too. I've been explicitly told to capture a settlement (by force), I've done so, and then I've been informed that another faction occupies the settlement. It doesn't happen often, but it does happen.

As for the first (blockade) mission, some of the blockade missions require you to blockade the port for three turns, if it was only blockaded for two turns, you ran out of time. Also, there is a difference in the mission to annex a settlement and take a settlement. If the nobles request you to annex a settlement, that's a diplomatic takeover. The other one is a request to just take it by military action. If you take a city with your armies and the mission was to annex you get the mission no longer necessary failure message.
