True. I add that a smilie should say a lot by itself already; there's no need for "text in balloons" to strengthen the expression already shown (unless there's a specific thing to be uttered, such as with the "Logic wins again" smilie).

While they all have their purposes -- the drama smilies -- the first one has the most utility not just with drama. Observe:

It can naturally be used in different dramatical contexts by the user, as already mentioned. And below are some more not necessarily dramatical.
