As HRE you can only recruit from Teutonic chapter guilds, but if there´s St. Johns built you still get public health bonus.
You can´t recruit any units from buildings you cannot build yourself, but if they give bonuses, you still benefit them. Try conquering Jerusalem from Egyptians in quite late game (let them have it till that) and enjoy Bimaristan.
EDIT: Forgot to ask, If there´s already St. Johns or Templars chapter house in settlement HRE takes, does it count as a guild preventing you from getting any other?

That was partly the point of my question...

It seems logical that any guild would count whether or not it isthe case but i would like a definitive answer on the matter
