Well, are you playing as a Muslim faction, I assume? If so, train some imams(you'll need a mosque for that) and put one in each province, which should convert people at a solid rate within your empire. In the meantime, you'll need some decent size garrissons until the province's loyalty goes up to at least 100%. Watchtowers, border forts, and town watches also help increase loyalty. Lastly, once you get a keep in the province, build a mosque which will help convert more people to Islam as well.

To increase command stars, you've got a few things. First of all, winning more battles. The more stars he has, the more battles he'll need to win to get the next star. Next, a few governor titles(notably Constantinople and Trebizond, although there are others) can give generals more command. Lastly, later on, you can build administrative buildings such as Admiralty, which will give you more titles to assign to generals which can give them more stars. Hope that helps you.

Now some more details:
- A spy (your own) can raise the loyalty in your province. If you put more than 1 spie in a province then the one with the highest valour counts.
- Your king/emporer/sultan also raises loyalty. Bring your sultan closer to the more rebelious provinces and their loyalty will go up. Just don't get him cut off from his sultanate (on an island without a port or behind enemy lines)
- The normal builing order for loyalty raising buildings is: watchtower => border fort (if you won't make to much use of spies and assasins on the offence) => fort => town watch.

Also do not forget to lower the taxes. This will improve province loyalty
Build all the chain churches you can there. (church, monastery, reliquary)
