From observation, I can say that quitting smoking on one's own is an exceptionally and scarcely successful undertaking, at least in such cases as yours, which resembles very much that of my uncle's. You may force yourself into not smoking and keep doing it for a long time. It will appear successful but that will be on the surface. Whenever you drop your guard (such as getting drunk at the new year's eve

) or encounter a hardship, you'll find yourself having smoked a few
last ones.
Instead of making it hard for yourself, go to a therapist who specialises in the area. There are plenty. With the inner support of your determination, he will help you quit it once and forever. Just find someone really good at it and whom you can trust. Prefer behavioural therapy and avoid medication or alternative chemicals; they don't work nearly as well.
And don't drop hope, don't give up, stand fast!
