I've seen it with knight in MTW and M2TW, but never in RTW.

Can't say I could recall ever witnessing it in action either. Probably dysfunctional and as such merely cosmetic.

probably bugged in RTW/M2TW engine. On Medieval 1 trying to babysit the knights was a big issue

I know I had a case or two in vanilla RTW, but clicking constantly some how reset them.

I've never had it either. However I've had plenty of units which refuse to withdraw from combat. This seems to have no correlation with the "impetuous" trait though.

It happened once or twice for me in vanilla, but since then not that I've noticed (they might have in the utter chaos full stack on full stack city fight though)

Im sure Ive seen cavalry and at least a couple of barbarian infantry units that were walking near an enemy suddenly charge of their own accord.
I suspect though it could be a case of where one man gets out of sync with his unit and then gets attacked, making the rest of the unit charge the enemy even if they are some distance away.

The only thing I've noticed about those "May charge without orders" units are that they don't freaking guard when I use guard mode! When a unit attacks them, more often than not, they start pushing back, meaning guard mode is basically useless to use with those guys. One of the reasons why I usually don't get those guys. I use guard mode a LOT as the advantages to stamina are terrific, and they just simply take it away.
Nowadays I stick those guys on the flanks.
